Both my mid and final site evaluations were conducted via zoom joined with 2 other classmates. I enjoyed being able to hear my classmate’s H&Ps so I could not only learn from them but also learn more about the location they were rotating at and the patient population at that site. During both site evaluations we were all asked a series of questions which tested our knowledge on psychiatric topics along with random facts/statistics that helped us better understand how psychiatric conditions exist today.
For my mid-site evaluation I presented 1 H&P. This H&P was on a young female who had overdosed on tylenol and was seen in the medical ER as a psychiatric consult before being admitted to CPEP for overnight observation. After presenting we discussed toxic levels of tylenol and treatment options for tylenol overdose. When discussing the plan for the patient we talked about different depression treatments and which drug options may be better/worse in a previous overdose patient. I was able to explain my differential diagnosis and why I ultimately landed on my final diagnosis.
For my final site evaluation I presented an additional H&P and a relating journal article. This H&P was similar to the first where it was also a suicide attempt patient. This patient was also seen as a consult in the medical ER before being admitted. For this case we discussed the importance of collateral and whether or not additional sources should have been contacted as collateral for this patient. We also discussed starting doses for SSRIs and how long they take in order for patient’s to start feeling their effects. For my journal article I presented an article that discussed suicide rates after psychiatric admission. This was relevant to me patient case because it discussed periods of times where suicide rates are highest after admission and also which groups may be more at risk. I was able to learn from both site evaluations and I was glad to be able to hear my classmate’s presentations.
