For my site evaluation we met on zoom and were and were able to present our patient cases and journal articles. For my H&P I presented a case on a patient who was diagnosed with cholelithiasis. I presented my HPI and spoke of my initial differentials and how I may help rule out/in the differentials. Since I included the lab work completed on the patient I was able to interpret the labs during my presentation and present the imaging findings that were found on ultrasound. I also included patient disposition as it is important in the ED to provide a disposition to know whether the patient will be admitted or discharged. I was provided with useful feedback on my presentation, along with on my other H&Ps. We were able to discuss cholecystitis vs cholangitis and speak of other patient cases where despite being discharged the patient returned with worsening symptoms.
I also presented a journal article relating to this topic which was a 2023 article in the Journal of the American Medical Association which compared the safety of robotic assisted cholecystectomy vs laparoscopic cholecystectomy. I spoke of the recent increase in use of robotic assisted surgeries, particularly for their precision. I also presented the results which showed that outcomes were better with laparoscopic cholecystectomies based on the outcome measurements. This allowed for the conclusion that laparoscopic cholecystectomies should remain the standard of care for cholecystectomies. During this visit I was also able to hear two classmates present their cases. Since they were at the same hospital it was interesting to see what type of cases they encounter at that location compared to the cases I was encountering at my location. I was able to learn from their case presentations and their articles.
